The American Public Health Association has put together various health fact sheets for states as well as for specific disparities. All fact sheets are accessible from the website and are offered in English and Spanish.

Chicago REACH program

The Lawndale project is part of REACH 2010, and was implemented to improve health in North and South Lawndale neighborhoods. The program targets individuals who are at risk for coronary heart disease and diabetes. The project works by conducting case management, community capacity-building, and outreach activities through a collaboration of community residents, hospitals, community agencies, academia and governmental agencies.

CDC Exemplary State Programs (Racial and Ethnic Approaches to Community Health)

This site gives examples of exemplary state programs that help to eliminate health disparities among minority populations. The list of programs was generated by the Center for Disease Control in an effort to give less progressive states examples of programs that work to close the disparities gap. The programs selected had to be based on strong research, demonstrate innovative approaches, and yield positive, measurable outcomes.

Operation Immunization

This is an immunization program to increase minority's awareness of the importance of immunizations and to raise the number of adults receiving immunizations. The program is run by the American Pharmacologic Association Academy of Students of Pharmacy and is implemented in 82 communities around the country.
